Here is detailed information about the form and how to use it.
Methods of Payment - Please send the completed form, and the necessary funds in the form of a Personal Check, Money Order, or Cashier's Check (in U.S. dollars) to the address at the bottom of the form. We no longer accept credit cards by mail. If you would like to use your credit card, please feel free to do so in our online store.
Estimated Time To Ship - Whichever method of payment you choose, after the funds have cleared, we will ship your order. Credit cards will be processed immediately upon receipt of your order. Please allow 3 - 5 business days for the other forms of payment to be processed. Allow an additional 3 - 5 business days for shipment of your order.
How to Use The Form - We would appreciate it very much if you fill out the form on your computer, and then print it. This will allow the calculation of your total amount due, and make it easier, and result in fewer errors on our part, due to the information being typed instead of handwritten.
By interactive, we mean that after you enter the item (or items) you would like to order, when you press the Order Total button, your payment amount will be calculated. If you wish to change the quantity or items you would like to order, just select the new information, press the Order Total button again, and your new total will be calculated. Note: We pay the Sales Tax for California residents!
After you enter your Billing Address, if your Shipping Destination is the same, just click in the Checkbox, and all your information will be duplicated and placed in the appropriate boxes.
Note: No information is sent over the internet using this method. The Order Total button is for your convenience and is used only to calculate the amount due. The form can even be filled out and printed after logging off the internet. After it is loaded into your browser, no connection to the internet is necessary to enter your ordering information.
Your Personal Information - If there is a question regarding your order and we need to contact you, the question can be resoved much more quickly if you give us a means, other than postal mail, to contact you.
